Man's Careless Driving Caused the Accident

A SOUTHAMPTON, England, man in his 20's was arrested and charged Friday morning with "suspicion of causing death by driving without due care and attention."

The Telegraph reports:

A female paramedic and an elderly patient were killed when an ambulance on a 999 call careered off a road and hit a tree.

The paramedic was driving the emergency vehicle when it ploughed into a ditch in the New Forest on Friday morning.

One of the elderly man's relatives and another paramedic, who were both in the back of the ambulance, suffered serious injuries in the crash on the A337 between Brockenhurst and Lyndhurst. The female paramedic and the elderly man were pronounced dead at the scene.

It is not yet known why the ambulance driver left the road.  There are no indications of an actual collision, but is possible that she was swerving to avoid one.  The investigation will continue through the day.

Head-On Crash On 2-Lane Road

A HEAD-ON CRASH NEAR DAVENPORT, IOWA, Thursday morning killed the driver of a car that collided with an ambulance around 7:30 am.

According to reports, the Medic EMS ambulance was transporting a patient non-emergency with two EMT's on board when the car, a Hyundai Tiburon, drifted across the center line of the 2-lane road and struck the ambulance.

The impact crushed the front end of the car under the ambulance and killed the car's driver.  None of the people on the ambulance were injured.

Two Units Crumpled

IN SUSSEX, ENGLAND, LAST WEEK a South East Coast Ambulance was involved in a fatal crash.  On Thursday February 28. one of their units collided with an auto in Brighton resulting in the death of the car's driver.

BBC News reported:

Emergency teams were called to Marine Drive outside Roedean golf club at about 22:40 GMT on Thursday.

Sussex Police said Gary Tucker, 45, died at the scene and his passenger was seriously injured. There were no patients in the ambulance but a crew member was injured, officers said. Mr Tucker, from Longridge Avenue, Saltdean, was driving a Fiat Seicento.

His passenger, a 43-year-old man, was taken to the Royal Sussex County Hospital with serious but not life-threatening injuries, police said.

The EMT was only slightly injured and was released after treatment.  The ambulance was responding on an emergency call at the time.

SIX DAYS LATER, Wednesday March 6, another South East Coast Ambulance was responding on emergency conditions and collided with another car, again in Brighton.  The crash caused the ambulance to leave the road and smash into an electric control box.

The two occupants of the car, a Peugot 106, were transported with whiplash and minor head injuries.

The two medics were shaken up, but did not require transportation.  There was no patient in the ambulance.  The cause of the crash has not been disclosed.

Driver of Other Vehicle Killed

A PRIVATE AMBULANCE OPERATING IN NEWARK, Ohio, was involved in a collision with a car this morning (Tuesday) when the car pulled our of a gas station into the path of the ambulance at 10:25 am.

The ambulance was carrying a patient on a non-emergency transport, no lights or siren, and struck the car on the left side.  The 92-yr.-old driver of the car was killed immediately.

One of the EMT's in the back of the ambulance received a serious head injury and was airlifted to a hospital in Columbus.  The other EMT, the patient, and the ambulance driver were all transported to another hospital in Newark with less serious but non-life threatening injuries.

Ambulance Was Not On a Call

A TRAFFIC ACCIDENT INVOLVING A MEADE COUNTY, Kentucky, ambulance Thursday evening left one woman dead and the two EMT's with serious injuries.  Two other people were also transported with less serious injuries.

The ambulance did not have a patient on board and was just driving non-emergency when a 27-yr.-old woman blew a stop sign and collided with the ambulance.  The ambulance driver saw the accident unfolding and took an evasive action which led to him glancing off another car.  But the crash was unavoidable and the ambulance struck the auto on the driver's side.  The ambulance then careened off the road and rolled over.

According to (Kentucky State Police) and Meade County Sheriff officials, a silver 2008 Pontiac driven by Sarah S. Hottell, 27, of Brandenburg was eastbound on KY 333 approaching the intersection of KY 313 when she failed to stop at a stop sign. An ambulance, driven by Todd Flory, 49, of Vine Grove swerved to Hottell's car but was unable to do so. The ambulance struck the Pontiac in the driver's side front, then left the roadway and overturned on the southbound side of the intersection ejecting both Flory and his passenger EMT Michael Wise, 48, from Payneville.

During the collision, a Volkswagen driven by Maria P. Williams, 47, of Brandenburg, was also struck.

Hottell's car also left the roadway on the south side and came to final rest in the ditch. She was pronounced dead at the scene. In addition to Hottell, a 10-month old infant, who was properly restrained in a car seat, was in the car.

One of the EMT's suffered a crushed pelvis and the other a serious back injury.

Three Killed in Ambulance

A BANGLADESHI PRIVATE AMBULANCE was struck by a loaded sand truck at 3:30 am Wednesday morning.  The crash killed the patient, his wife and the ambulance driver.  Three other passengers were injured and were transported.

The Dhaka Daily Star is reporting:

Three people including a couple were killed and three others injured as a truck crashed into an ambulance at Sher-e-Bangla Nagar in the capital early Wednesday.  Rafiqul Islam, a sub-inspector (SI) of Sher-e-Bangla Nagar Police Station, told The Daily Star that a sand-laden truck heading for Farmgate from Gabtoli hit an ambulance at the intersection in front of Bangabandhu International Conference Centre around 3:30am.

Ambulance passengers Pathan, 50, and his wife Farzana Pathan, 40, and driver Ansar Ali, 35, died on the spot, said Bozlar Rahman, another SI of the police station.

The accident took place when the family members were shifting Nilu Pathan, who was undergoing treatment at an Uttara hospital for his heart problem, to another hospital at Mirpur by the rented ambulance after his condition deteriorated, SI Rafiqul said.

The injured — Nilu Pathan's son Sajib, 22, and his two cousins Manik and Liton — were rushed to Dhaka Medical College Hospital where condition of Sajib stated to be critical.  The bodies were also sent to the DMCH morgue for autopsy.

The driver and helper managed to flee the scene leaving the truck on the scene.

A CHERAW, SOUTH CAROLINA, BASED AMBULANCE was involved in a traffic accident with a tractor-trailer around 11 am Tuesday morning.  The crash killed the EMT in the passenger seat and hospitalized the driver.  The accident occurred in front of the Carolinas Medical Center in Monroe, North Carolina.

Update:  The driver of the ambulance was Jonathon Corey Brown of Cheraw.  He was transported to a hospital in Charlotte where he is in stable condition.  The EMT who died was Belinda Gale Rivers, 43, also from Cheraw.  The truck driver was Wallace Kenneth Murphy of Madison and he was uninjured. 

The State Police say that the truck was traveling west on U. S. 74 and the ambulance was coming out from a cross street and making a left turn when the two vehicles collided.  WSOC-TV Ch. 9 is reporting that the ambulance was coming out from a side street when it collided with the truck.  The EMT who died was thrown out and pinned underneath the ambulance.

The investigation is continuing and as of Tuesday evening no charges had been placed.
Update #2, Friday am:  The Associated Press is reporting:

A South Carolina ambulance driver has been charged in a wreck that killed a co-worker near a North Carolina hospital earlier this week.  Jonathan Cory Brown of Cheraw, SC, was charged with misdemeanor death by vehicle on Thursday.

Brown was driving the ambulance that pulled in front of a tractor trailer rig on U.S. 74 in Monroe late Tuesday morning.

The Sandhills Ambulance Service transport ambulance had just left off a patient and was returning to Cheraw at the time.

Ambulance Driver Was Retired EMS Lieutenant

A BMW UTILITY VEHICLE careened out of control on a Staten Island, New York, street around 7 pm Monday night and slammed head-on into an ambulance.  The crash killed both drivers, with the SUV driver ejected in the crash and dying on the scene.  The EMT-driver of the Staten Island University Hospital ambulance was pinned in the cab and had to be extricated.  He died later at the hospital.

The EMT was David Restuccio, 58, who was a retired  EMS Lieutenant who worked for the FDNY from 1981 to 2006.  The other EMT was in the back of the ambulance with a patient and both of them were transported with minor injuries.

Witnesses told police that the car was traveling at a high rate of speed when it hit the median strip and "flew" across into the oncoming lanes where it impacted the ambulance head-on.

Perp Was Fleeing From Another Accident

A GALLATIN, KENTUCKY, MAN WAS KILLED Tuesday when the ambulance he was being transported in was struck head-on by a Jeep SUV.

The Gallatin County Fire/EMS Department ambulance was carrying the patient on a non-emergency transport for a dialysis treatment when the Jeep coming the opposite direction came across the center line and struck the ambulance "at highway speed," causing it to flip onto its roof.

The two firefighters on the ambulance were injured and had to be transported.  The miscreant driver was trapped in his wrecked car and had to be extricated before being taken to the hospital.  The patient, a 59-yr.-old man died a short time later at the hospital from his injuries.

The ambulance driver tried to pull off the two-lane road but could not avoid being hit head-on by the jeep SUV.

Kentucky state police identify the jeep driver as 22-year old Hunter Legrand of Warsaw. "At this time, preliminary investigation indicates drugs were involved."

A few minutes before hitting the ambulance, police say Legrand drove his car off of Johnson Road, a few miles away. He then knocked down a utility line connector and actually knocked some wires down. Kentucky Tpr. Brad Aterburn says, "The Gallatin County dispatcher had received a previous call of reckless driving. The driver had hit a telephone pole and a mailbox on a different road. He was well across the center line on this road. I'm not sure if speed was a factor, though."

And Guess Who Had the Red Light?

IN OGDEN, UTAH, A NORTH DAVIS FIRE DISTRICT ambulance sailed through a red light Friday morning and struck an auto that was crossing the intersection with the green light.  The collision killed a passenger in the car.

After smacking the auto, the ambulance glanced off and struck two more cars that were in the oncoming lanes and had stopped for the light.  The accident occurred around 10:45 am.  The ambulance was transporting a patient using its lights and siren.  Besides the patient, there were four EMT's on board the unit.  Two of the EMT's were transported with broken bones and contusions.  Their patient had been in an auto accident 30 minutes earlier and was already in critical condition from her first wreck of the day.  This accident created further injuries for the patient.

A total of eleven people were involved in this multi-vehicle crash and six of them were transported.

South Ogden Police Officer Marci Edwards said an investigation is under way to determine who was at fault for the accident. "There are policies and procedures when running lights and sirens," she said. "There will be an investigation as to whether they were entering the intersection properly."

Retired Firefighter Killed

THE DRIVER OF A CAMPBELL COUNTY, VIRGINIA, AMBULANCE was charged today (Friday) with reckless driving following a crash in Lynchburg Thursday afternoon.

The crash occurred at a controlled intersection when the ambulance drove through a red light and collided with a pickup truck, killing the truck's driver, Dean Anders, 69, a retired Lynchburg firefighter who died on the scene.  The pickup had already entered the intersection when the ambulance came through and struck the truck on the passenger side causing it to flip over.

The Lynchburg News and Advance reports:

A witness at the scene said she was on Campbell Avenue heading into the city, when the ambulance passed her.

Roseann Dickerson, of Lynchburg, said she saw the ambulance slow approaching the light, but continued through the intersection where it hit the pickup. At one point, the ambulance was sitting partially on top of the truck, she said.

Capt. Ryan Zuidema, of the Lynchburg Police Department, said the ambulance was transporting a patient to Lynchburg General Hospital.

That patient, a family member, Kidd and another Campbell County emergency worker staffing the ambulance were taken to Lynchburg General Hospital. None are believed to have life-threatening injuries.

Driver and Patient Both Killed

A TRANSPORT AMBULANCE WAS INVOLVED in a deadly collsion Wednesday morning.  The ambulance appears to have been operated by only the driver who was taking a patient who was in a wheelchair from one facility to another.  The accident occurred near Chillicothe, Ohio, around 10 am.

Update:  The Chillicothe Gazette has the updated description of how the accident occurred:

According to Sgt. Kari Riebesell of the Ohio Highway Patrol, Carl Dearth, of Frankfort, was headed south in the flatbed truck on Hospital Road when he slowed and stopped to turn left onto Delano Road. (The ambulance) was also headed south on Hospital, and Riebesell said he failed to stop in time, striking the back of Dearth’s truck with a full-frontal impact just before 10 a.m.

The lumber on the back of Dearth’s truck went through the ambulance’s windshield and halfway through the ambulance, causing King and McWhorter to suffer fatal head injuries. Both were pronounced dead at the scene by Ross County Coroner Dr. John Gabis.

Three Vehicles Involved

A MEDTRANS AMBULANCE IN PUEBLO COUNTY, Colorado, was involved in a 3-vehicle accident Thursday afternoon that proved fatal to the ambulance patient.

The accident took place on a 2-lane highway and was triggered by the ambulance.  A Xcel Energy pickup truck had slowed to make a left turn when the ambulance coming from behind it failed to slow down and, while trying to evade a collision, veered to the right shoulder but crashed the left front of the ambulance into the right rear of the pickup.

The ambulance continued about 30 yards before rolling over into a ditch.  The patient, an 84-year-old man suffered multi-trauma injuries and was flown to the hospital where he later died.

The pickup truck that was struck was knocked into the oncoming lane and collided head-on with an SUV which in turn flipped over into the ditch on the other side of the highway.

The two passengers in the car and the two ambulance workers were all transported with non-life threatening injuries.  The driver of the pickup was apparently uninjured.

Non-Emergency Transport Turns Deadly

A PRIVATE AMBULANCE IN VIRGINIA COLLIDED head-on with a cement mixer truck Friday afternoon, leaving the driver of the ambulance dead and fatally injuring the patient.  The EMT in the patient compartment was seriously injured and is in stable condition.  The driver of the cement truck was uninjured.  The ambulance operated by Priority Ambulance Service was transporting an 82-yr.-old man from the University of Virginia Hospital in Charlottesville to a nursing home.  The accident occurred south of Charlottesville in Buckingham County shortly after noon.

The Virginia State Police issued the following statement:

At 12:20 p.m. Virginia State Police Trooper T.C. Edmondston was called to the scene of a two-vehicle fatal crash on Route 20 near the intersection of Route 699 in Buckingham County.

Two people were killed and one person was seriously injured in the crash. The ambulance was traveling southbound on Route 20 when it crossed the centerline into the northbound lane. A cement-mixing truck was unable to avoid the oncoming ambulance and the two vehicles collided.

The impact of the crash caused the ambulance to overturn onto its side. The driver of the ambulance, Timothy K. Southern, 21, of Waynesboro, died at the scene. Southern was wearing a seat belt. A medical technician, Heath P. Davis, and patient, Howard L. Watson, 82, of Dillwyn, were riding in the back of the ambulance.

Both were flown to UVA Hospital in Charlottesville. Davis, 24, is listed in stable condition. Watson died at the hospital later Friday. The driver of the cement mixer, John W. Dorman II, 29, of Charlottesville, was not injured in the crash. Dorman was wearing a seat belt. The Virginia State Police Crash Reconstruction Team responded to the scene to assist with the investigation, which remains ongoing at this time.

No charges will be filed. Route 20 was re-opened by 5:15 p.m.

Ambulance Driver Charged With Negligent Homicide

Career-Ending Crash for Wrong-Laning Driver

THE EMSA AMBULANCE DRIVER in Oklahoma City who was involved in a fatal collision on December 10 has lost his job and been charged by police with negligent homicide.  (See Firegeezer video report on the accident HERE.)

Benjamin Ward Samples, 36, was charged Tuesday for the crash that resulted in the death of a man after Samples had driven in the oncoming lane at a high rate of speed to bypass traffic that was stopped ahead.  Investigators say that Samples moved over into the wrong-lane while responding to an emergency call.  The driver of the car was also in the left lane and began making a legal left turn which put him in the path of the ambulance.  The accident investigation revealed that Samples was traveling at 83 mph in a 40 mph zone.

State law allows an ambulance to travel at a speed not over 10 mph above the posted limit.  EMSA's rules prohibit driving faster than 15 mph when wrong-laning.

Going Through a Red Light

A DODDRIDGE COUNTY, WEST VIRGINIA, AMBULANCE was involved in a crash Sunday night that killed the patient they were carrying.

The wreck occurred on Rte. 50 near Clarksburg when the ambulance traveled through a red light and collided with a pickup truck.  The impact knocked the ambulance on its side and the patient, William Wiseman, 53, was pronounced dead after arriving at the hospital. 

The ambulance had its lights and siren operating, but the police have stated that the ambulance did not have legal control of the intersection.

One other person was also transported for minor injuries.

SUV Broadsides the Ambulance

A PATIENT DIED WHEN A HAMPTON, VIRGINIA, AMBULANCE was struck at an intersection by an SUV around 8 pm Saturday night.  The impact knocked the ambulance onto its side and left 7 people injured including 4 members of the Hampton Fire Department.

The ambulance was running lights and siren with a 42-yr.-old cardiac patient along with 3 FD medics and a student on board.  All four medics were transported along with three people from the SUV.  The patient died later at the hospital.  Two of the medics were kept overnight and released Sunday.

Police say that the SUV struck the ambulance broadside after failing to yield the right of way and witnesses are saying that they saw the ambulance stop at the intersection before it began crossing through..  They have not yet determined who was the cause of the collision.

Three Firefighters Injured, One Civilian Dead

EARLY SUNDAY MORNING, A FIRE DEPARTMENT ambulance responding on a call was struck by a private automobile in Cambrai, France.

The ambulance was manned by three Cambrai firefighters who were all injured in the 3 am crash.  One of the firefighters, age 36, suffered a possible fractured neck and was transferred to a major hospital in Lille.  The other two FF's, ages 23 and 28, were treated and released later Sunday morning.

The car had three passengers and one of them, a 25-yr.-old woman, was killed in the wreck.  The car's driver had minor injuries and another passenger was seriously injured and remains hospitalized.

Deadly Collision in Germany

A 69-YR.-OLD WOMAN was killed Tuesday night when her car was struck by an ambulance near Passau, Germany.

The woman was waiting in a left-turn lane when the ambulance approached from behind with its warning lights and siren activated.  While most of the cars waiting at the intersection pulled aside to the right to let the ambulance pass, it appears at first that the woman pulled to the left instead.  The 21-yr.-old ambulance driver applied the brakes immediately, but the collision occurred anyway as the ambulance crashed into the left side of the car just behind the driver's seat.

The woman was trapped in her car and had to be extricated by the rescue squad before she could be transported by helicopter to the hospital where she died later last night.

The local prosecutor has commanded both vehicles to be seized, as is routine, while the police conduct an investigation into what happened to cause the accident.
